Create Initial Disclosure

  1. Launch your Internet browser software and open Coeus Lite http://mv.ezproxy.com.ezproxyberklee.flo.org/coeus.
  2. From the Coeus Home page, click the “My COI” tab to open the COI Home Landing Page.
  3. Click the Initial COI Disclosure link in the Create column on the left side of the page. The four step disclosure process will begin.

NOTE: If you exit the disclosure before completing it, your Initial disclosure will now be In Progress. You can return at a later time to update this as a Disclosure in Progress, described in a later section.
